I just tested the form and it worked fine. I was using Firefox. What browser are you using? If you continue to have an issue please let me know. I can submit your entry manually as a last resort.

The form doesn't work for me…

I fill it in and click Ok, a “view log” button lights up… if I click that “view log” it says
“Error occured please contact application owner.”
